Employees Only: COVID-19 Self-Screening

To keep all our employees safe and in accordance with Governor Murphy's Executive Order 192 we are going to be requiring that every employee complete a self-assessed questionnaire for COVID-19 symptoms and risk factors each day before entering our facilities, effective November 5, 2020.

The survey must be completed prior to your shift.  After completing all of the questions one of the following codes will appear:

• GREEN check mark clearing you to work
• or a RED STANDBY phone icon with instructions to call your supervisor. 

Before you submit, make sure to capture proof of the green check mark that you receive at the end of the survey, as you will be expected to show proof of clearance upon arriving at work. If you are answering the questionnaire on your mobile phone take a screenshot before submitting; or if you are completing it on a computer, print the check mark before you press submit.  Show the check mark image to your supervisor when you come into work.    

Regardless of survey results, if you feel that you have symptoms related to COVID-19 please contact a healthcare professional.

The survey should be completed prior to your arriving at work on all days you are scheduled to work in our facilities until further notice.
